TransCare is a specialist service designed to support NHS teams in the UK with the safe and effective transition of patients from hospital back into the home or community. Working in partnership with NHS teams, we provide tailored clinical pathways that help to ensure continuity of care and reduce hospital stays, prioritising quality outcomes and overall care experience.

We are available on all bank holidays except Christmas Day, Boxing Day and New Year’s Day. Specific schedules will be communicated before Christmas.
The pharmacy team will review internal and external stability data.
Under the HPN framework, this is 5 working days for new/existing formulations; urgent changes may be possible in 3 or 4 working days.
